If your health insurance is accepted by our office and you have supplied insurance information, we will bill your insurance carrier for you.

Patient co-payments are due at the time of your visit.

Medicaid and CHIP plans require a patient to have a referral form from their primary care physician prior to receiving specialist treatment.
